A Statutory Declaration is a formal document, which is sworn as an oath in the presence of either a solicitor or magistrate. A Statutory Declaration declares that you will abandon your former name and adopt a new name from then onwards.

In the UK the most widely used document as evidence of a change of name is a Deed Poll. However, if you are living abroad or are not a British Citizen you may be asked to produce a Statutory Declaration to change over some of your documents.
